Payment Options
For your convenience, we accept Visa, MasterCard, American Express and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 616-949-2720. Many times, a simple telephone call will clear any misunderstandings or questions regarding your account.

Financial Policy
We will send you a monthly statement. Most insurance companies will respond within four to six weeks, so you will receive a statement after they reimburse us if they did not pay the full amount we estimated for you at your visit. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Missed payments and balances over 30 days are subject to finance charges of $5.00 per month for the first 90 days, then $25.00 per month thereafter. Missed or broken appointments without 24 hours notice will incur charges based on the length of scheduled appointment time (minimum charge of $25.00).
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
